About the Role
As a Support Coordinator at Safe Hands, you will be the primary point of contact for a dedicated caseload of NDIS participants across Newcastle and the Hunter region. You'll help participants understand and implement their NDIS plans, connect them with appropriate providers, and coordinate the supports they need to live the life they choose.
This is a full-time position that requires strong organisational skills, genuine empathy, and a solid understanding of the NDIS framework. You'll work closely with participants, their families, and a wide network of service providers to ensure supports are in place, working well, and aligned with each participant's individual goals.
Key Responsibilities
- Build and maintain trusting, professional relationships with NDIS participants and their families or carers
- Assist participants to understand the purpose and scope of their NDIS plans and funding categories
- Connect participants with appropriate service providers, community resources, and informal supports
- Coordinate multiple supports across health, housing, community, and daily living domains
- Develop and review Support Coordination Progress Reports for the NDIA as required
- Assist participants to prepare for and actively participate in NDIS plan reviews
- Monitor and manage budgets across support categories; alert participants to underspend or overspend risks
- Maintain accurate, timely case notes and records in line with NDIS practice standards
- Respond to participant concerns, crises, or changes in circumstance promptly and professionally
- Liaise with allied health professionals, planners, Local Area Coordinators, and other stakeholders
- Identify risks and escalate safeguarding concerns in accordance with mandatory reporting obligations
- Participate in regular supervision, team meetings, and continuing professional development
Essential Requirements
You must meet the following requirements to be considered for this role:
- Minimum 2 years' experience in disability, community services, social work, or a related field
- Demonstrated knowledge of the NDIS - including plan categories, price limits, and practice standards
- Strong written communication skills; ability to produce clear, professional reports
- Excellent time management and the ability to manage a caseload independently
- Proficiency with digital tools - email, Microsoft Office, case management software
- High level of empathy, cultural sensitivity, and commitment to person-centred practice
Desirable (Not Essential)
- Tertiary qualification in Social Work, Disability Studies, Community Services, or a related discipline
- Previous experience in an NDIS Support Coordination role
- Experience working with participants with complex support needs (psychosocial, behaviour support, etc.)
- Knowledge of local Newcastle/Hunter region service providers
- Experience using NDIS portals, myplace, or Careview/Brevity
